What Causes Dental Implant Failure?

Implant failure is usually grouped into early failure (before the implant fully bonds to the jawbone) and late failure (months or years after placement). Common causes include:

  • Poor osseointegration: The implant doesn’t fuse properly with the bone, sometimes due to inadequate bone density or movement during healing.
  • Infection (peri-implantitis): A bacterial infection around the implant can damage the gum tissue and bone, leading to loosening of the implant over time.
  • Smoking or vaping: Nicotine reduces blood flow and slows healing, increasing infection risk.
  • Uncontrolled medical conditions: Diabetes and other conditions that impact healing can raise complication rates.
  • Grinding or bite issues: Bruxism and an uneven bite can place excess force on the implant.
  • Poor oral hygiene: Plaque buildup around the implant can trigger inflammation and bone loss.

Warning Signs

Some discomfort is normal after surgery, but certain symptoms warrant a call to your dentist:

  • Persistent pain that doesn’t improve
  • Swelling, redness, or pus around the implant
  • Bleeding when brushing near the implant
  • Receding gums or a “longer” looking tooth
  • Mobility or looseness (an implant should not wiggle)
  • Trouble chewing or a bite that suddenly feels “off”
  • Bad taste or chronic bad breath that won’t go away

How Dental Implant Failure Is Treated

If caught early, your dentist may recommend deep cleaning around the implant, antimicrobial rinses, or antibiotics to control infection. Bite adjustments or a night guard may be used if clenching and grinding are a factor.

If the implant is loose or bone loss is significant, the implant may need to be removed to allow the area to heal. In many cases, patients can still have a successful implant after bone grafting or additional healing time.
